Acute Plan

This focuses on one acute issue or co-morbidities. Our nurse works with the patient to produce a plan, which can be used to advise the emergency department or St Johns what the patients main issues are and their management plan. We are currently offering plans for patients with asthma, COPD, heart failure, diabetes or mental health issues.
